Dental exams are by appointment for active duty members and are required annually within 30 days of your birthday. Appointments for dental cleanings are encouraged to be scheduled at six month intervals. 

Dental Care for Remote Units:
United Concordia Dental network of providers is used for Active Duty personnel only.  Please call United Concordia ADDP at (866) 984-2337 to acquire a dental access control number (ACN). This number is needed to schedule an appointment with a network provider. 

Dental Care for Dependents:

Eligible family members must use a United Concordia Dental provider.  A list of available Dentist may be found at: https://www.uccitdp.com/find-a-dentist/ or you may call (844) 653-4061 to speak to a United Concordia Dental customer service representative.
