USCG Base Kodiak Pass & ID
Base Kodiak Personnel Services Division is responsible for issuing ID cards and DEERS maintenance.

Note: Walk-Ins are only accepted for locked, broken, or lost ID cards.
Monday - Thursday: 8:00 AM - 11:00 AM, 1:00 PM - 3:00 PM (Closed for Lunch from 11:00 -13:00)
Friday: 8:00 AM - 11:00 AM (Closed for Cleaning/Machine Maintenance from 11:00 - 16:00)
Saturday - Sunday: CLOSED
CLOSED on all Federal and National Holidays and for Command Sponsored Liberty.
Sponsor's can complete the following self-service tasks online using at https://www.dmdc.osd.mil/self_service/:
- Generate and sign a DD Form 1172-2, Application for Identification Card/DEERS Enrollment, which family member to receive his or her replacement ID card without being accompanied by the Sponsor.
- Update Sponsor and/or Sponsor family members’ contact information (e.g., address, email, phone number).
- Request a family member be added to her/his DEERS record (e.g., child, spouse, parent/parent-in-law).
- Activate CAC certificates.
- Add a Personnel Category Code (PCC) to her/his CAC.
- Download applets to her/his CAC.

ID Cards Issuance - All Customers:
- Two unexpired forms of ID
- One must be state or government-issued photo ID
- Acceptable examples include driver's license, passport, or Social Security card.
Adding a Spouse:
- Marriage certificate
- Spouse's birth certificate
- Spouse's Social Security card
- Spouse's Government Issued ID
- All documents must be originals or certified copies
Adding a Child:
- Child's Birth certificate
- Child's Social Security card
- If child was acquired through marriage, the marriage certificate is also required
Children over age 21, Full-Time Student Status:
- Letter from the school's registrar's office stating they are enrolled full time in an accredited college in pursuit of an Associate's Degree or higher to have benefits extended to age 23.
Child Born out of Wedlock (Female Sponsors):
- Child's Birth certificate
- Child's Social Security card
Child Born out of Wedlock (Male Sponsors):
- Child's Birth certificate
- Child's Social Security card
- Court order establishing paternity or a State Voluntary Acknowledgement of Paternity form
Dependent Parent:
- DFAS/PPC (LGL) letter of approval
- Service member birth certificate
- Service member's Government Issued ID
- Parent's government issued ID
- Parent's Social Security
Dependent Ward:
- DFAS/PPC (LGL) letter of approval
- Court document placing the child in the member's household for one year or more
- Ward's Birth certificate
- Ward's Social Security card
- All documents must be translated in English and certified
TRICARE Young Adult:
- Applies to children age 21-26
- Cannot be married or eligible for employer-sponsored health insurance
- Once enrolled in TRICARE Young Adult, individual is eligible for an ID card.
- Must have two valid forms of ID and proof of enrollment in TRICARE Young Adult
- Acceptable examples include driver's license, passport, or Social Security card.
Beneficiaries Turning Age 65 (TRICARE For Life):
- Medicare card
- Two forms of ID
Removing SSN from ID Cards:
- Current cards will not be replaced until your card is 30 days to its expiration date.
- If your card has an INDEF expiration date, make an appointment for a new card.
