MILITARY ID CARDS AND HEALTH RECORDS: Patients must bring their military identification card and health record with them to the clinic (if the record is not maintained at the clinic). Your Military ID card indicates your eligibility for care. Your health record contains important information about you and is a critical source of information to the provider rendering your medical care.

Health Insurance Portability & Accountability Act (HIPAA) effective 14 April 2003: By now most of you have probably been given the Notice of Privacy Practices and have been asked to sign something indicating you have received it. The HIPAA Notice of Privacy Practices describes how the Juneau Clinic staff may use or disclose your protected health information (PHI), with whom that information may be shared, and the safeguards we have in place to protect it. This notice also describes your rights to access and amend your PHI. You have the right to approve or refuse the release of specific information outside of our system except when the release is required or authorized by law or regulation. If you would like a copy of the Notice of Privacy Practices please see the Juneau Clinic front desk staff.

Retiring or separating from the service? If you plan to retire or are being released from active duty (RELAD) soon, please call to schedule a physical exam at 463-2140. This physical may be done up to a year in advance of retirement or separation. This physical is however, recommended to be done within six months of retirement/RELAD, so that all medical and dental conditions may be documented on the physical.

Dental Clinic Services: The dental clinic provides care to active duty only. Active duty members are strongly encouraged to enroll their dependents in a dependent dental care program.

Retirees and their families are encouraged to enroll in the TRICARE Retiree Dental Program. This is open to National Guard/Reserve in Retired Reserve status, including retirees under age 60. For more information call 888-838-8737 or go to www.trdp.org.

Reservists can visit the clinic for their annual dental exam. Full dental services are available for reservists on active duty for 30 or more continuous days.

Individual Medical Readiness is the extent to which an individual active duty or reserve is free from health related conditions that could limit their ability to fully participate in Coast Guard operations. All active duty and reserve Coast Guard Personnel are required to be medically ready for deployment.

Medically Ready: To be fully medically ready a member must meet all six of the criteria:

1. 1. Periodic Health Assessments: The congressionally mandated Periodic Health Assessment has replaced the Quinquinnial Physical Exam. The Juneau Clinic still offers specialty physicals such as Pre-commissioning, RELAD, Separation, and retirement, but instead of the 5 year PE we are now requiring the Annual Periodic Health Assessment.
2. 2. Dental: Service member must be rated as either Class 1 or Class 2 or failed if they are deemed Class 3 or Class 4. Class 1 means not requiring dental treatment or revaluation within 12 months. Class 2 means a service member has an oral condition that, if not treated or followed up, has the potential to, but is not expected to result in emergencies within 12 months. Class 3 signifies that a service member has an oral condition that if not treated is expected to result in a dental emergency within 12 months. Class 4 means a service member needs a dental examination.
3. 3. Medical Equipment: Service members who need glasses are required to have at least 2 pairs of corrective lenses with them during any deployment.
4. 4. Medical Readiness Labs: Service members are required to have a DNA specimen on file, baseline Tuberculin Skin Test (TST), sickle cell test and G-6-PD screen on file and an HIV test within 24 months.
5. 5. Immunizations: The following immunizations are required:
   • - Hepatitis A - two shot series
   • - Hepatitis B - three shot series
   • - Influenza - annually
   • - MMR - once in your career
   • - OPV/IPV - once in your career
   • - Tetanus-diphtheria - every 10 years
   • - Yellow fever - alert force members
   • - Smallpox - operational units
6. 6. No Deployment Limiting Conditions: Are you Fit for Full Duty (FFD)?

Medical Readiness does not mean you are deployable. Being deployable also includes other factors determined by the Commanding Officer such as core competencies in job skills, but medically ready individuals are deployable from a medical standpoint and that is the goal with Medical Readiness. It is the Commanding Officer decision whether to deploy members that do not meet CG Medical Readiness criteria.

Periodic Health Assessments are required annually during your birth month. To schedule a physical exam contact the Juneau Clinic Front Desk at 907-463-2140.

Procedures: To ensure your physical exam runs smoothly and to limit the amount of time spend filling out forms in the clinic, follow these procedures;

1. You must fast for 12 hours prior to your physical exam. NO EATING! You can drink water and are encouraged to drink lots of it. Urine and blood samples may be necessary.
2. If you wear glasses be sure to bring them to your appointment - please do not wear contacts.
3. You will need your Health Record (medical/dental record) with you on the day of your appointment.

• OMSEP

Occupational Medical Surveillance and Evaluation Program (OMSEP) refers to a physical examination program established to monitor the health of Coast Guard personnel working in jobs designated as having high health risk potential to chemical or physical agents. The purpose of the program is to enable the Coast Guard to anticipate, and thereby minimize occupational illness among our members. In coordination with environmental health field professionals, information from the OMSEP is used to modify operations so that employee exposure is minimized. OMSEP also serves as a historical repository of information on personnel exposed to potentially hazardous environmental conditions and is used for studies directed toward a better understanding of the causes and mechanisms of prevention.

Hearing Conservation Program (HCP): Noise exposure is by far the most common occupational exposure in the Coast Guard, warranting a special program known as the Hearing Conservation Program (HCP) under the umbrella of the OMSEP. The purpose of the HCP is to identify the onset of a hearing Threshold Shift before the individual experiences hearing loss sufficient to interfere with verbal communication. Early detection of hearing loss allows aggressive protective action to be taken before permanent hearing loss occurs.

Optometry Services are not available at the Juneau Clinic. A list of local providers can be obtained from the local TRICARE representative at 1-888-874-9378.

The Juneau Clinic utilizes UNICOR as an alternative to the local providers for obtaining eyeglasses. It takes approximately two to three week to receive the finished glasses.

TTOHC is funded travel at government expense to obtain authorized, medically necessary, non-emergency, specialty health care to a provider more than 100 miles from the Primary Care Manager (PCM) IAW 10 USC §1074, JFTR U7960 and HSWLSUPACTINST 6000.1.

After your Primary Care Manager has submitted a medical referral to TRICARE and you have been issued a TRICARE authorization number, you may submit a request for medical travel orders using the TTOHC Request Form.

Cascade of Care Policy: All health care referrals are required to be issued utilizing the nearest participating network provider as determined by the TRICARE contractor.

You must file a claim within three days of completing travel. Your claim must include the original travel order (this letter) authenticated and signed by an Authorizing Official as "Certified to be a true copy", travel voucher DD Form 1351-2, all receipts for air fare, lodging, meals and authorized purchases.
