Please arrive 15minutes early to each appointment in the Uniform of the Day

If you show up to your appointment without ALL of your required paperwork, we reserve the right to reschedule your appointment

If you check in 10minutes (or later) past your scheduled appointment, you will be completed as a NO SHOW

**After hours care is not a substitution for a regularly scheduled appointment**

Base Elizabeth City medical clinic maintains on-call services 24 hours a day to address urgent care needs.All active duty personnel are responsible for contacting their team Duty HS or Medical Officer after hours prior to seeking urgent/non-life threatening care.

EMERGENCY CARE – GO TO THE NEAREST EMERGENCY ROOM ASAP.

As soon as it is practical, notify the Duty HS and Command of the emergency room care and/or admittance to the hospital.

 

PATIENT CENTERED WELLNESS HOME

A Patient Centered Wellness Home is a team of healthcare providers and support staff who deliver services for total healthcare, surrounding the patient with support to accomplish wellness goals. This healthcare model will include and coordinate the disciplines of medicine, dentistry, pharmacy, family support, work-life, and safety, forming the Patient Centered Wellness Home. Each patient is expected to actively participate in developing individualized health and wellness directives and goals with their assigned team.

 

ROUTINE CARE

As part of the Patient Centered Wellness Home, the Base Elizabeth City Clinic provides primary care services for active duty personnel. Each active duty member will be assigned to a team that will attend to your healthcare needs. Active duty members can be seen for acute care by walk-in, on a space available basis. Routine care will be seen by appointment only and may be scheduled by calling or visiting the clinic Front Desk.

Occupational & Aviation Medicine · Immunization· Lab Work· Primary Dental Care · Prescription Services ·Over the Counter Medications· Minor surgery·Referral services
